Has anyone ever had a PCP cylinder failure?

As an aside to @Steve636 s thread on an exploding PCP, I want to ask if anyone has firsthand experience (i.e. actually had it happen in their own presence) of a PCP cylinder failing whilst it be used or stored normally?
Not because, or while, it was being worked on.
 
There was a gent on this forum who had a daystate that did go bang.
I think his username was xcudlaty but I may have got that not quite correct.
 
Apart from the obviously messed with gun in the other thread, I had an A&M customs cylinder crack.. not dramatic this time but that had never been dropped, overfilled or buggered about with in any way...

Is an A&M customs cylinder an aftermarket cylinder or gun original?
 
No but have read many reports of it happening .

The Daystate one could have been damage to the rifle by a previous owner . the Fitting sheared and it could have been dropped or stressed at some point .

The Owner of GNIB lost some fingers and seriously ill in hospital but I don't know the full story on that one .

A Sportsmatch rifle had one blow when being filled if I remember right . Pics of it are online . @rockdrill knows a bit about it if I remember right. wrong spec of materials I believe .

Unless you were there or know the history of the cylinder , You won't know what has gone on to cause it .

Yes, I do know a bit about that incident, I wasn't there when it went, my knowledge of it comes from the late Tom Walton, who owned the rifle and was holding when the failure occurred during filling.

Tom gave me the picture below when I visited him to buy the stainless steel GC2 that Sportsmatch built for him.

He told me about the incident and the follow up investigation which identified that some non-seamless tubing had been supplied in error, hence Sportsmatch went back and performed ultrasound scans and pressure tests to check for any others in which the wrong material had been used.
